Abstract Medicines must be subject to physical, chemical, and biological analysis to guarantee their quality, safety, and effectiveness. Despite the efforts to ensure the reliability of analytical results, some uncertainty will always be associated with the measured value, which can lead to false decisions regarding conformity/non-conformity assessment. This work aims to calculate the specific risk of false decisions regarding conformity/non-conformity of acetaminophen oral solution dosage form. The acetaminophen samples from five different manufacturers (A, B, C, D, and E) were subject to an active pharmaceutical ingredient assay, density test, and dose per drop test according to the official compendia. Based on measured values and their respective uncertainties, the risk values were calculated using the Monte Carlo method implemented in an MS Excel spreadsheet. The results for two acetaminophen oral solution samples (C and D) provided an increased total risk value of false acceptance (33.1% and 9.6% for C and D, respectively). On the other hand, the results for the other three acetaminophen samples (A, B, and E) provided a negligible risk of false acceptance (0.004%, 0.025%, and 0.045% for A, B, and E, respectively). This indicates that measurement uncertainty is very relevant when a conformity assessment is carried out, and information on the risks of false decisions is essential to ensure proper decisions.

O estudo apresentado neste artigo analisou características de notícias falsas e de rumores falsos, que têm como principal diferença a incorporação, ou não, do que caracteriza a imprensa oficial e a grande mídia. A pesquisa avaliou material sobre a pandemia de covid-19 no Brasil, entre janeiro e dezembro de 2020, utilizando o banco de dados do projeto Latam Chequea Coronavirus, que continha 838 mensagens falsas disseminadas em mídias sociais, além de blogs e sites de notícias, e, após submetidas a critérios de corte, 473 delas foram analisadas. A análise utilizou 33 categorias previstas na literatura, divididas em: caracterização, tipo, tema e técnicas de persuasão. Os resultados revelaram que: 86,89% das mensagens eram rumores; os temas mais frequentes foram "profilaxia e cura", "políticas" e "prevenção pública"; o apelo à autoridade esteve presente em 76,74% delas e foi a técnica de persuasão mais frequente, seguida do apelo emocional (64,90%). A pesquisa mostrou como as técnicas de persuasão empregadas apoiam os temas identificados nas mensagens falsas e rumores falsos sobre a pandemia.


The study presented in this article analyzed characteristics of fake news and false rumours, differentiated by the incorporation or absence of features of official press and of the media. The research evaluated mate-rial about the covid-19 pandemic in Brazil, between January and December 2020, using the database from the Latam Chequea Coronavirus project, containing 838 false messages disseminated on social media, as well as blogs and newsbrands, and after being subjected to cut-off criteria 473 of them were analysed. The analysis employed 33 categories outlined in the literature, divided among characterization, type, theme, and persuasion techniques. The results revealed that: 86,89% of the messages were rumors; the most frequent themes were "prophylaxis and cure", "politics", and "public prevention"; the appeal to authority was present in 76,74% of the messages and it was the most frequent persuasion technique, followed by emotional appeal (64,90%). The research has demonstrated how persuasion techniques support the themes of fake news and false rumors regarding the pandemic


El estudio presentado en este artículo analizó las características de las noticias falsas y de los rumores falsos, cuya la principal diferencia es si incorporan o no lo que caracteriza a la prensa oficial y a los principales medios de comunicación. La investigación evaluó material sobre la pandemia de covid-19 en Brasil, entre enero y diciembre de 2020, utilizando la base de datos del proyecto Latam Chequea Coronavirus, que contenía 838 mensajes falsos difundidos en redes sociales, además de blogs y sitios de noticias, y después de ser sometidos a criterios de corte se analizaron 473 de ellos. El análisis utilizó 33 categorías previstas en la literatura, divididas en: caracterización, tipo, tema y técnicas de persuasión. Los resultados revelaron que 86,89% de los mensajes eran rumores; los temas más frecuentes fueron "profilaxis y cura", "políticas" y "prevención pública"; la apelación a la autoridad estuvo presente en 76,74% de ellos, y fue la técnica de persuasión más frecuente, seguida de la apelación emocional (64,90). La investigación muestró como las técnicas de persuasión respaldan los temas de noticias falsas y rumores sobre la pandemia

ABSTRACT

O atestado odontológico compreende um tipo de documento emitido pelo profissional que expressa a veracidade de um fato relacionado ao paciente constatado durante a prática clínica. O objetivo deste artigo foi apontar e analisar o tipo de crime que incorre o cirurgião-dentista que emite atestado falso. Trata-se de uma pesquisa transversal com abordagem descritiva realizada por meio do levantamento das provas de concursos públicos realizados no Brasil para o cargo de Perito Odontolegista ou nomenclatura correspondente. Foram considerados os concursos realizados nas unidades federativas do Brasil sem distinção quanto ao ano, sendo que a coleta das informações se deu nos meses de janeiro a abril de 2023. Conclui-se que, apesar de ocorrerem discrepâncias de interpretação muito pontuais na literatura odontológica quanto ao condizer entre os Artigos 299 e 302 do Código Penal Brasileiro, não há possibilidade legal de o cirurgião-dentista ser sujeito ativo do crime de falsidade de atestado médico, por ser crime próprio do profissional médico


The dental certificate comprises a kind of document issued by the professional that expresses the veracity of a fact related to the patient verified during clinical practice. The aim of this article was to point out and analyze the type of crime incurred by dentists who issue false certificates. This is a cross-sectional research with a descriptive approach carried out through the survey of public tenders held in Brazil for the position of Forensic Odontologist or corresponding nomenclature. It was considered the contests held in the Brazilian Federative Units without distinction to the year, and the information collection was performed from January to April 2023. It can be concluded that, although there are few interpretation discrepancies in the dental literature regarding articles 299 and 302 of the Brazilian Penal Code, there is no legal possibility for the dentist to be an active subject of the crime of falsity of medical certificate, as it is a crime specific to the professional medical doctor

ABSTRACT

Chat Generative Pre-Trained Transformer (ChatGPT) has revolutionized how we perceive artificial intelligence (AI).: Judge Juan Manuel Padilla Garcia created history by mentioning its use while passing judgment about an autistic child and payment for his medical treatment by his insurance company. The use of AI is not new and is helping the judiciary system in many ways. However, this judgment given on January 30, 2023, has ignited controversy among Judge Garcia’s peers and the global community (a Google search produced more than 70 million hits on February 5, 2023). EU has established guidelines that are to be followed before calling any AI tool trustworthy. This requires stringent compliance with the verification and due diligence process. In this instance, ChatGPT was used within 2 months of its launch, even when it has been shown to give incomplete, incorrect, and misleading answers in many instances. Hasty adaption of unproven technology, however good it may be, should not be our path. This might fuel the misguided fear amongst people about robots taking over from human judges.

ABSTRACT

Background: Correct mapping of the blind spot is important, as it serves as an estimate of fixation reliability. When the blind spot is not seen in the expected location in Humphrey visual field (HVF) printout, the clinician should give a thought to why this might be the case. Purpose: This video describes a series of cases, in which due to different reasons the blind spot could not be seen in the presumed expected location in the grayscale and numeric data of the HVF printout and the possible explanation behind this. Synopsis: When interpreting perimetry results, it is important to know whether the field test is reliable or not. A stimulus presented at the location of physiologic blind spot should not be seen by a patient with a steady fixation in Heijl– Krakau method. Responses will also occur, however, if the patient has a tendency for false?positive responses, or when the blind spot of the properly fixing eye is not in the location where the test stimulus is presented, because of anatomic variation, or if the patient's head is tilted while performing the test. Highlights: Perimetrist should recognize these potential artifact, during the test and relocate the blind spot. In case, such results are seen after finishing the test, it is recommended for the clinician to repeat the test

ABSTRACT

Objective:To compare the clinicopathological features of patients with false negative and true negative pathological biopsy of sentinel lymph nodes in endometrial cancer, and to explore the related factors of missed diagnosis of sentinel lymph nodes.Methods:From January 2020 to January 2022, 31 patients underwent sentinel lymph node biopsy combined with systematic lymph node resection in the First Affiliated Hospital of Shandong First Medical University were retrospectively analyzed, of which 2 were false negative and 29 were true negative. PubMed literature on sentinel lymph node false negative of endometrial cancer was searched from the establishment of the database to December 2022, with the search terms "Sentinel lymph node" "Endometrial neoplasms" and "False negative" . A total of 15 cases of false negative patients with similar methods to this study were extracted. In the false negative group, there were 17 false negative patients with sentinel lymph node negative but systemically excised lymph node positive, including 2 cases in our hospital and 15 cases in the literature. The true negative group included 29 true negative patients with negative sentinel and systemic lymph nodes, all from our hospital. The clinicopathologic features of the two groups were compared.Results:There were statistically significant differences in tumor grade ( χ2=6.09, P=0.014) , lymph vascular space invasion ( P=0.012) and myometrial invasion ( χ2=9.66, P=0.002) between the two groups. However, there was no significant difference in histological type between the two groups ( χ2=0.19, P=0.661) . Conclusion:There is a risk of false negative for sentinel lymph node biopsy in patients with endometrial carcinoma with high-grade tumor, myometrial invasion ≥1/2 and lymph vascular space invasion.

ABSTRACT

Objective To investigate the causes of false-positive results in the 68Ga-labeled fibroblast activation protein inhibitor (68Ga-FAPI-04) PET/CT imaging. Methods The imaging data of 547 patients undergoing 68Ga-FAPI-04 PET/CT examination in the Department of Nuclear Medicine of the Affiliated Hospital of Southwest Medical University from September 2020 to May 2021 were retrospectively collected.Two experienced nuclear medicine diagnostic physicians analyzed the clinical data,relevant imaging examinations,laboratory examinations,pathological results and follow-up results of the patients with false-positive results. Results The 68Ga-FAPI-04 PET/CT imaging of 547 patients showed false-positive results in 99 (18.1%) patients,including 56 males and 43 females.The postoperative pathological examination confirmed false-positive results in 13 patients,including 1 patient of thyroiditis,2 patients of pulmonary tuberculosis,1 patient of bone tuberculosis,2 patients of pulmonary inflammatory pseudotumor,1 patient of pulmonary sarcoidosis,1 patient of pulmonary benign fibroma,1 patient of organic pneumonia,2 patients of renal angiomyolipoma,1 patient of mass pancreatitis,and 1 patient of pancreatic mucinous cystadenoma.The medical history,relevant imaging examination,and long-term follow-up confirmed false-positive results in 86 patients.Specifically,the false-positive uptake in the neck,chest,abdomen,bone joint,and skin occurred in 8 (9.3%),13 (15.1%),5 (5.8%),57 (66.3%),and 3 (3.5%) patients,respectively.Inflammation-related uptake appeared in 83 (83.8%) patients with false-positive imaging results,of which arthritis (23 patients) and osteophyte (29 patients) were the most common.Sixteen (16.2%) patients showed the false-positive uptake related to fibroblasts. Conclusion 68Ga-FAPI-04 PET/CT imaging will show non-malignant tumor false-positive results,which are mainly associated with inflammation and fibroblasts.

Resultados falsos positivos na mamografia de rastreamento são comuns a essa intervenção e trazem ônus para as mulheres e o sistema de saúde. O objetivo deste estudo foi estimar o risco de resultado falso positivo no rastreamento mamográfico brasileiro com base em dados de sistemas de informação do Sistema Único de Saúde (SUS). Foi realizado estudo de coorte histórica de mulheres de 40-69 anos, que realizaram mamografia de rastreamento e exame histopatológico de mama no SUS, nos anos de 2017 a 2019. A taxa de resultados falsos positivos foi estimada a partir da prevalência de resultados BI-RADS alterados na mamografia de rastreamento e da proporção de resultados benignos no exame histopatológico de mama. Das 10.671 mulheres com exame histopatológico no SUS, 46,2% apresentaram resultado benigno, sendo essa proporção significativamente maior em mulheres de 40-49 anos comparada à de mulheres de 50-69 anos. A estimativa de resultados falsos positivos foi de 8,18 casos por 100 mulheres na faixa etária de 40-49 anos, e de 6,06 por 100 mulheres na faixa de 50-69 anos. Essas informações são úteis aos gestores na avaliação de programas de rastreamento do câncer de mama, assim como aos profissionais de saúde, para que orientem a mulher sobre as implicações do rastreamento mamográfico.


False-positive results on mammography screening are common, putting a burden on both women and the health care system. This study aimed to estimate the risk of false-positive results in Brazilian mammography screening based on data from the Brazilian Unified National Health System (SUS) information systems. A retrospective cohort study was conducted with women aged 40-69 years, who underwent mammography screening and breast histopathological examination at SUS from 2017 to 2019. The rate of false-positive results was estimated based on the prevalence of altered BI-RADS results on mammography screening and the proportion of benign results on breast histopathological examination. Of the 10,671 women with histopathological examination at SUS, 46.2% had a benign result, and this proportion was significantly higher in women aged 40-49 years compared to women aged 50-69 years. The estimate of false-positive results was 8.18 cases per 100 women aged 40-49 years and 6.06 per 100 women aged 50-69 years. This information is useful for public managers in evaluating mammography screening programs, as well as for health care providers to guide women on the implications of mammography screening.


Los resultados falsos positivos en la mamografía de cribado son comunes en esta intervención y suponen prejuicios para las mujeres y el sistema de salud. El objetivo de este estudio fue estimar el riesgo de resultados falsos positivos en el cribado mamográfico brasileño a partir de los datos del sistema de información del Sistema Único de Salud (SUS). Se realizó un estudio de cohorte histórica de mujeres de 40-69 años, que se sometieron a mamografía de cribado y examen histopatológico de mama en el SUS, de 2017 a 2019. Tetradenia riparia (Hochst.) Codd (Lamiaceae) is a shrub, commonly known as ginger bush or false myrrh, and several studies have shown that T. riparia exhibits a variety of biological properties. This study aimed to determine the chemical composition of T. riparia essential oil and its fractions, investigate their anticholinesterase activity, and assess their larvicidal activity against the cattle tick Rhipicephalus microplus and the mosquito Aedes aegypti. Eleven essential oil fractions were obtained by fractionation and analyzed by gas chromatography/mass spectrometry. Larvicidal activity against R. microplus and third-instar A. aegypti was assessed using a larval packet test and a larval immersion test, respectively. Anticholinesterase activity was determined by a bioautographic method. Forty-nine compounds were identified in the essential oil, of which the major classes were oxygenated sesquiterpenes (45.95%) and sesquiterpene hydrocarbons (35.20%) and the major components were isospathulenol (17.40%), ß-caryophyllene (15.61%), 14-hydroxy-9-epi-caryophyllene (10.07%), 14-hydroxy-α-muurolene (8.32%), and 9ß,13ß-epoxy-7-abietene (5.53%). Bioassays showed that T. riparia essential oil (LC50 = 1.56 µg/mL) and FR3 (LC50 = 0.30 µg/mL) were the most active against R. microplus and A. aegypti larvae, respectively. The essential oil and FR1, FR2, and FR3 exhibited acetylcholinesterase inhibitory activity. These results indicate that T. riparia essential oil and its fractions hold promise in the development of novel, environmentally safe agents for the control of R. microplus and A. aegypti larvae.

ABSTRACT

Este artigo apresenta como principal objeto de estudo a falsa acusação de abuso sexual no contexto da alienação parental para, diante dela, estabelecer a seguinte problemática: será possível propor uma eventual correlação entre si e os processos psíquicos do luto e da melancolia? Neste sentido, a partir do recurso teórico ao referencial psicanalítico de Freud e de Laplanche, debate as circunstâncias que norteiam o discurso levado ao Judiciário pelo genitor alienante valorizando em tal movimento não apenas a realidade material da prova, tão importante no campo jurídico, mas também a realidade psíquica ditada pelo inconsciente, a qual se pauta em uma noção de verdade que, na sua vinculação direta com a particularidade de cada sujeito e com o dinamismo das relações específicas que ele estabelece consigo mesmo e com os outros, coloca em xeque as certezas positivistas da norma. Em termos conclusivos, destaca o quanto, a despeito da atual literatura existente sobre alienação parental no Brasil a correlacionar, em regra, a um luto mal elaborado por parte do alienante, é possível e mesmo desejável cogitar também a presença da melancolia - ou, mais especificamente, de traços melancólicos intermediários - na formação e desenvolvimento desse fenômeno.(AU)


This article presents as the main object of study the false accusation of sexual abuse in the context of parental alienation, to establish the following problem: would it be possible to propose a probable correlation between parental alienation and the psychic processes of mourning and melancholia? In this sense, based on the psychoanalytic theoretical framework of Freud and Laplanche, the article discusses the circumstances that guide the discourse taken to the judiciary branch by the alienating parent, valuing in such action not only the material reality of the evidence, which is very important in the legal field, but also the psychic reality dictated by the unconscious, which is guided by a notion of truth that, in its direct connection with the particularity of each subject and with the dynamism of the specific relations that they establish with themselves and others, threatens the positivist certainties of the norm. In conclusive terms, it highlights how, despite the current existing literature on parental alienation in Brazil generally correlates it to a poorly elaborated mourning by the alienating person, it is possible and even desirable to also consider the presence of melancholia-or, more specifically, of intermediate melancholic traits-in the formation and development of this phenomenon.(AU)


Este artículo presenta como principal objeto de estudio la falsa acusación de abuso sexual en el contexto de alienación parental, con el fin de responder al siguiente planteamiento: ¿Es posible proponer una posible correlación entre la alienación parental y los procesos psíquicos de duelo y melancolía? Para ello, desde el marco psicoanalítico de Freud y de Laplanche, se discuten las circunstancias del discurso llevado al Poder Judicial por el padre alienante, que valora en tal movimiento no solo la realidad material de la prueba, tan importante en el campo jurídico, sino también la realidad psíquica dictada por el inconsciente, el cual se guía por una noción de verdad que, en su conexión directa con la particularidad de cada sujeto y con el dinamismo de las relaciones específicas que establece consigo mismo y con otros, pone en jaque las certezas positivistas de la norma. En la conclusión, destaca cómo, a pesar de la literatura actual existente sobre la alienación parental en Brasil, en general, la correlaciona con un duelo mal diseñado por parte de la persona alienante, es posible e incluso deseable considerar la presencia de la melancolía -más específicamente, de rasgos melancólicos intermediarios- en la formación y desarrollo de este fenómeno.(AU)

ABSTRACT

A abordagem transradial é indicada para reduzir risco de morte, complicações vasculares ou sangramento. Em pacientes com doença cardíaca isquêmica estável, a abordagem radial é recomendada para diminuir sangramento no local de acesso e complicações vasculares. As complicações são raras, podendo ocorrer hematoma, perfuração e, muito raramente, pseudoaneurisma da artéria radial. Neste relato de caso, é descrito um caso raro de pseudoaneurisma da artéria radial esquerda associada à síndrome compartimental no antebraço esquerdo após realização de cateterismo eletivo. O diagnóstico foi confirmado por ultrassonografia com Doppler, que evidenciou pseudoaneurisma da artéria radial esquerda, medindo 2,1x1,5cm, com colo de 0,3cm. O tratamento cirúrgico do pseudoaneurisma e da síndrome compartimental restaurou a função completa do membro.


The transradial approach is indicated to reduce the risk of death, vascular complications, or bleeding. In patients with stable ischemic heart disease, the radial approach is recommended to reduce vascular complications and bleeding on access site. The complications are rare, and hematoma, perforation, and pseudoaneurysm (very rarely) of the radial artery may occur. This case report describes a rare case of left radial artery pseudoaneurysm associated with compartment syndrome in the left forearm after elective catheterization. The diagnosis was confirmed by Doppler ultrasonography, which showed a left radial artery pseudoaneurysm, measuring 2.1x1.5cm, with a 0.3-cm long/wide neck. Surgical treatment of pseudoaneurysm and compartment syndrome restored full limb function.

METHODS Through the investigation of the sales form of Chinese medicinal materials on the e-commerce platform and the search of Chinese medicinal materials online dispute cases, the difficulties and existing problems in the supervision of Chinese medicinal materials under the new online sales model were analyzed, and corresponding countermeasures were proposed. RESULTS & CONCLUSIONS Clarifying the conceptual boundary between Chinese medicinal materials and their decoction pieces, agricultural products and food is the key to standardizing the online sale of Chinese medicinal materials. The regulatory criteria, which determine whether Chinese medicinal materials belong to drug management based on whether they had been included in medicinal channels, didn’t consider the diversity of online sales channels for Chinese medicinal materials, their safety and the disguised sales of Chinese herbal decoction pieces. It is necessary to establish the concept of hierarchical management of Chinese medicinal materials, strictly restrict the behavior of claiming the efficacy of Chinese medicinal materials and selling Chinese herbal decoction pieces in live streaming, improve the defining path of false propaganda of Chinese medicinal materials, implement the responsibilities and obligations of live streaming marketing subjects and platforms, and safeguard the legitimate rights of consumers.

ABSTRACT

Lateral flow immunoassay (LFIA) is a rapid detection technique that allows researchers to move the antigen-antibody reaction from a test tube or laboratory vessel to a test strip. Due to the chromatographic effect of the test strip, the solution would move to a specified direction based on the test and complete the whole antigen-antibody specific reaction. A qualitative judgment can be made with the naked eye by observing the color change of the reagent strip at a specific location. Because of its advantages of being fast, simple, specific, inexpensive, and requiring no specialized personnel, LFIA is now widely used in medical testing, food quality monitoring, environmental monitoring, agriculture and animal husbandry. A major bottleneck for the development of LFIA technology is the hook effect. This paper summarizes the current methods, means and research progresses to combat the hook effect, hoping to provide a strong technical reference for researchers to design test strips, select suitable nanoparticles, and achieve quantitative LFIA detection.

ABSTRACT

Objective:To evaluate the value of modified magnetic bead screening for enrichment of cell-free fetal DNA (cffDNA) in non-invasive prenatal testing (NIPT).Methods:This study retrospectively recruited 31 cases with low concentration of cffDNA (<6.00%), Z value in the gray zone (3.00-4.00) at the first detection, or false-positive (confirmed by invasive prenatal diagnosis) or false-negative (confirmed by postnatal chromosome test) results among 11 000 pregnant women who underwent routine NIPT in Beijing Haidian District Maternal and Child Health Care Hospital from October 2017 to December 2019. Plasma samples collected for the first-time routine NIPT were used to enrich cffDNA using modified magnetic beads for NIPT (modified NIPT). Wilcoxon rank sum test was used to compare the modified NIPT with the routine NIPT in detecting the cffDNA concentrations of male fetuses.Results:Among the 31 pregnant women, there were 13 cases with low cffDNA concentration in routine NIPT, 11 having false-positive results in the routine NIPT (three for trisomy 13, four for trisomy 18 and four for trisomy 21, all were confirmed by invasive prenatal diagnosis), six with gray-zone Z values in the first-time NIPT (retesting indicating low risk) and one having false negative result for trisomy 21 (confirmed by postnatal chromosome test). Cell-free DNA (cfDNA) fragments less than 150 bp were effectively enriched using the modified magnetic bead screening and the concentration of cffDNA was increased from 4.43% (2.45%-17.61%) in routine NIPT to 13.46% (7.75%-36.64%) in the modified NIPT ( Z=-14.22, P<0.01). Results of the modified NIPT indicated that 13 cases with low cffDNA concentration of routine NIPT were successfully detected as low risk, as well as the risks in the six cases with gray-zone Z value and six of the 11 false-positive cases in the routine NIPT were low, which were consistent with the retest results of the routine NIPT, while high risk was found in one false-negative case. Conclusions:The modified NIPT could reduce the false positive rate by lowering the failure rate caused by low concentration of cffDNA and is able to identify false-negative cases. Compared with the routine NIPT, it shows a higher success rate and a lower false positive rate.

Background: While performing the visual field test, some patients press the response button, at random and not in response to seeing a stimulus, resulting in a classical trigger happy visual field. Purpose: This can be identified by 10 pointers as shown in this single field analysis printout of Humphrey visual field (HVF), 24?2, Swedish interactive threshold algorithm (SITA) standard test of the right eye. These points are useful teaching points for the students to recognize. Synopsis: The video describes classical 10 points, which can be identified in an HVF 24?2 printout in a patient with high false?positive (FP) error. Highlights: The FP index is the most useful of the reliability indices. In SITA strategy, the FP rate is based upon the assessment of patient’s reaction time over the entire course of the test, and hence is not calculated until the test is completed. Therefore, it is important for the perimetrist to be alert to recognize the signs early during the test, such as high fixation los s (despite good fixation as seen on screen monitor) and unusually high retinal sensitivity values (up to 50 dB), and reinstruct the patient. If the test is completed, it should be repeated. This is important as the FP rates >15% are associated with compromised test results and are automatically removed from the glaucoma progression analysis.

ABSTRACT

Presentamos el caso de un varón de 64 años quien, tras un primer episodio de pancreatitis aguda necrotizante, reingresa a los 20 días por cuadro de dolor epigástrico intenso y posteriormente episodio de hemorragia digestiva alta en forma de hematemesis y melenas con inestabilización hemodinámica. Se realiza en ese momento gastroscopia urgente objetivándose probable fistula gastrointestinal en bulbo duodenal con coágulo adherido sin sangrado activo en ese momento por lo que se realiza angio-TC urgente que revela colección peripancreática necrótica con presencia de sangrado activo en su interior, procedente de la arteria pancreatoduodenal. La arteriografía urgente identificó imagen compatible con pseudoaneurisma arterial dependiente de la rama de arteria pancreatoduodenal, que fue embolizada con éxito. Desgraciadamente el paciente falleció en las horas posteriores, como consecuencia de un shock séptico secundario a colección pancreática infectada.


We present the case of a 64-year-old man who, after a first episode of acute pancreatitis, was readmitted 20 days later due to severe epigastric pain and later an episode of upper gastrointestinal bleeding in the form of hematemesis and melena with hemodynamic instability. An urgent gastroscopy was performed at that time, revealing a probable gastrointestinal fistula in the duodenal bulb with an adherent clot without active bleeding at that time, so an urgent CT angiography was performed that revealed a necrotic peripancreatic collection with the presence of active bleeding inside from the pancreatoduodenal artery. Urgent arteriography identified an image compatible with arterial pseudoaneurysm dependent on the pancreaticoduodenal artery branch, which was successfully embolized. Unfortunately, the patient died a few hours later as a result of septic shock secondary to an infected pancreatic collection.

Abstract Objective The immediate referral of patients with risk factors for placenta accreta spectrum (PAS) to specialized centers is recommended, thus favoring an early diagnosis and an interdisciplinary management. However, diagnostic errors are frequent, even in referral centers (RCs). We sought to evaluate the performance of the prenatal diagnosis for PAS in a Latin American hospital. Methods A retrospective descriptive study including patients referred due to the suspicion of PAS was conducted. Data from the prenatal imaging studies were compared with the final diagnoses (intraoperative and/or histological). Results A total of 162 patients were included in the present study. The median gestational age at the time of the first PAS suspicious ultrasound was 29 weeks, but patients arrived at the PAS RC at 34 weeks. The frequency of false-positive results at referring hospitals was 68.5%. Sixty-nine patients underwent surgery based on the suspicion of PAS at 35 weeks, and there was a 28.9% false-positive rate at the RC. In 93 patients, the diagnosis of PAS was ruled out at the RC, with a 2.1% false-negative frequency. Conclusion The prenatal diagnosis of PAS is better at the RC. However, even in these centers, false-positive results are common; therefore, the intraoperative confirmation of the diagnosis of PAS is essential.


Resumo Objetivo Recomenda-se o encaminhamento imediato de pacientes com fatores de risco para espectro placentário acreta (PAS, na sigla em inglês) para centros especializados, favorecendo assim o diagnóstico precoce e o manejo interdisciplinar. No entanto, erros diagnósticos são frequentes, mesmo em centros de referência (CRs). Buscou-se avaliar o desempenho do diagnóstico pré-natal para PAS em um hospital latino-americano. Métodos Um estudo descritivo retrospectivo incluindo pacientes encaminhados por suspeita de SAP foi realizado. Os dados dos exames de imagem do pré-natal foram comparados com os diagnósticos finais (intraoperatórios e/ou histológicos). Resultados Foram incluídos 162 pacientes no presente estudo. A idade gestacional mediana no momento da primeira ultrassonografia suspeita de PAS foi de 29 semanas, mas as pacientes chegaram ao CR de PAS com 34 semanas. A frequência de resultados falso-positivos nos hospitais de referência foi de 68,5%. Sessenta e nove pacientes foram operadas com base na suspeita de PAS com 35 semanas e houve 28,9% de falso-positivos no CR. Em 93 pacientes, o diagnóstico de PAS foi descartado no CR, com frequência de falso-negativos de 2,1%. Conclusão O diagnóstico pré-natal de PAS é melhor no CR. Entretanto, mesmo nestes centros, resultados falso-positivos são comuns; portanto, a confirmação intraoperatória do diagnóstico de SAP é essencial.

ABSTRACT

A falsa coletivização é um fenômeno crescente no mercado brasileiro de planos e seguros de saúde. Ela decorre diretamente de decisões regulatórias que afetam o setor, especialmente as diferenças entre regras aplicáveis a contratos individuais e coletivos. O objetivo deste trabalho foi analisar a evolução recente do fenômeno sob três aspectos: (i) a expansão desse tipo de contrato, simultânea à redução de planos individuais; (ii) o diferencial acumulado de reajustes para os falsos coletivos; (iii) a sua judicialização e o tratamento dado ao tema pelo Poder Judiciário. Foram utilizados dados da Agência Nacional de Saúde Suplementar, desagregados por empresa, entre 2014 e 2019; do banco de dados de Notas Técnicas de Registro de Produto da agência, entre 2015 e 2019; e dados primários produzidos pela análise de acórdãos do Tribunal de Justiça de São Paulo, proferidos em 2018 e 2019. Os resultados evidenciaram o crescimento do fenômeno dos "falsos coletivos", associado à gradual redução dos planos individuais. Demonstraram, também, o sistemático reajuste das mensalidades acima do teto definido pela Agência Nacional de Saúde Suplementar para planos individuais. A análise de acórdãos mostrou que o entendimento da questão pelo Poder Judiciário não é uniforme, nem em termos do resultado do julgamento, nem de sua fundamentação jurídica. Os resultados apoiam a interpretação de que esses contratos permitem às operadoras burlar aspectos relevantes da regulação do setor, impor reajustes superiores e, quando reclamadas judicialmente, escamotear a legislação consumerista.


False collectivization is a growing phenomenon in the Brazilian health insurance market, stemming directly from regulatory decisions that affect the sector, especially the diferences between the rules applicable to individual and collective contracts. Hence, this paper sought to analyze the recent evolution of this phenomenon under three aspects: (i) expansion of this type of contract, simultaneous to the disappearance of individual private health plans; (ii) premium increases for "false collectives"; (iii) its judicialization and treatment in the jurisprudence. Data was collected from the Brazilian Regulatory Agency for Private Health Insurance and Plans, detailed by company, between 2014 and 2019; the agency's Product Registration Technical Notes database, between 2015 and 2019; and primary data produced by analyzing rulings by the São Paulo Court of Justice, issued in 2018 and 2019. Results show the growth of "false collectives," associated with the gradual disappearance of individual private health plans. They also demonstrate the gap between premium increases and the ceiling set by the Brazilian Regulatory Agency for Private Health Insurance and Plans for individual private health plans. Analysis of the rulings reveal that the Judiciary's understanding on the matter is not uniform, neither in terms of the outcome, nor of its legal reasoning. These findings support the interpretation that such contracts allow insures to circumvent relevant aspects of the sector's regulation, to impose higher premiums and, when contested in court, to evade consumer legislation.

Resumen (analítico) El objetivo del estudio fue describir conocimientos y mitos de profesores colombianos sobre Altas Capacidades Intelectuales (ACI), analizando su relación con variables sociodemográficas de interés. Se utilizó una metodología no experimental, transversal descriptiva-co-rrelacional. Participaron 584 profesores de preescolar, educación básica y media, de 160 municipios seleccionados mediante muestreo no probabilístico, que diligenciaron el cuestionario de conocimientos y mitos sobre ACI. El porcentaje general de aciertos en el cuestionario fue bajo: se encontraron niveles elevados de falsas creencias relacionadas principalmente con la naturaleza de la superdotación, su rendimiento, sus logros y sobre la intervención educativa. De igual manera, se encontró relación entre las falsas creencias sobre ACI y el género de los participantes. Estos resultados ponen de relieve la necesidad de mejorar la formación sobre ACI en la preparación del profesorado.


Abstract (analytical) The objective of this study was to describe the knowledge and myths among Colombian teachers regarding students with High Intellectual Abilities (HIA), analyzing their relationship with sociodemo-graphic variables of interest. A non-experimental, descriptive-correlational cross-sectional methodology was used. 584 preschool, basic and secondary education teachers from 160 municipalities were selected through non-probabilistic sampling for participation and completed the HIA Knowledge and Myths Questionnaire. The general percentage of correct answers in the questionnaire was low and there were high levels of false beliefs primarily related to the nature of giftedness, the academic performance and achievements of these students and suitable educational interventions for this population. A relationship was found between false beliefs about HIA and the sex of participants. These results highlight the need to improve HIA training in teacher preparation.


Resumo (analítico) O objetivo do estudo foi descrever os conhecimentos e mitos de professores colombianos sobre Altas Capacidades Intelectuais (ACI), analisando sua relação com variáveis sociodemográficas de interesse. Foi seguida uma metodologia transversal descritiva-correlacional não experimental. Participaram 584 professores de educação infantil, fundamental e médio de 160 municípios selecionados por meio de amostragem não probabilística, que responderam ao Questionário de Conhecimento e Mitos da ACI. O percentual geral de respostas corretas no questionário foi baixo, havia altos níveis de falsas crenças relacionadas principalmente à natureza da superdotação, seu desempenho e realizações, e sobre a intervenção educacional. Foi encontrada uma relação entre falsas crenças sobre ICA e o sexo dos participantes. Esses resultados destacam a necessidade de melhorar o treinamento do ACI na preparação de professores.
